COTS & Partnership Community Health Center Celebrate One Year Anniversary!

It is the one year anniversary of the Partnership Community Health Center (PCHC) clinic at COTS being open, and what a year it has been! In the past year the clinic saw 171 patients, completed 530 appointments including 82 physicals, and assisted 31 individuals to gain insurance or prevented loss of coverage!  Patients came from COTS, Riverview Gardens™, Ryan Community, and the Warming Shelter. Amazing work to help our residents move to self-sufficiency!
With a focus of addressing the root causes of homelessness and working with PCHC to open the clinic, we are able to directly address one of the most significant barriers for our residents: health, including physical, behavioral and/or dental. By “Bringing Health Care Home”, we are able to give people the resources necessary to dramatically improve their well-being. During our May Community Dinner, which doubled as an anniversary celebration for the clinic, residents were invited to share their “Bright Spots” on the positive effects of the clinic’s presence. The responses were incredible. Residents revealed the wide range of substantial life improvements they’ve been able to make. These healthy new beginnings proclaim a brighter future for our residents!
